Welcome to C-DIAS.ORG (“we,” “us,” or “our”). C-DIAS.ORG is operated and maintained by the Center for Dissemination and Implementation at Stanford University School of Medicine.
Information We Collect
“Personally Identifiable Information” (or “PII”) refers to contact information that directly or indirectly tells specifically who you are, such as your name, phone number, company name, job, email, job title, research projects, postal address, or any other information that could be linked to you with reasonable effort. This information may be collected when you voluntarily provide it to us.
We do not collect sensitive information such as location information (GPS), political opinions, religious or philosophical beliefs, genetic or biometric data, data concerning health or data concerning sexual orientation.
We may automatically collect, store, and use information about your interactions and usage with our website, such as the pages you visit, the links you click, and the time and date of your visits, to enhance your user experience with our website, and for C-DIAS to better assess and enhance its programming, resources, and services to meet its mission.
How We Collect and Use Your Information
We collect PII directly from you when you voluntarily complete a “Subscribe,” “Contact Us,” or “Application” form, and when you request that we send you emails or resources. Before collecting your PII, you will have the option to give or withhold your consent for our sending you emails including our use of your PII for marketing emails to promote our resources, programming, and services. You may withdraw your consent at any time by unsubscribing to our emails by using an opt-out link contained in every email.
We may make full use of your Usage Data (for example email bounces, clicks on hyperlinks in emails, web page visits, and web downloads) for purposes such as to customize email communications and to analyze the performance of those emails, as well as your interaction with our website. Subject to your right to opt-out, we may combine Usage Data with PII that we collect, and use the combined data for purposes of improving our resources, programming, and services and your user experience with our website.
When We Act as Controller
We will be acting as the “Controller” of the personal data you provide directly to us via a “Subscribe,” “Contact Us,” or “Application” form, when creating an account, signing up for email communication, applying, or via other contact methods. You are not required to provide your personal information for purposes of navigating through the public areas of our website.
How Long We Keep Your Data
We will not retain your PII or Usage Data for longer than required. This means that we will keep your PII or Usage Data for any or a combination of time periods described as follows: (i) until we no longer have a valid reason for keeping it, (ii) until you request us to stop using it, or (iii) up to 10 years.
Information Sharing and Disclosure
We will not share or sell your PII with any outside parties. We may provide your PII and Usage Data to affiliated C-DIAS organizations. Except for sharing your information as described above, we will not disclose information about you or the contents of your communications unless compelled to do so in the good-faith belief that such action is reasonably necessary (a) to comply with the law; (b) to comply with legal process; (c) to enforce these terms and conditions; or (d) to respond to claims that any content violates the rights of third parties.
